You have been assigned a user name and password. Once you enter your user name and password at the top of this screen, click on “Log-in” and the next screen will require you to change the password when first signing on. Please refer to the Frequently Asked Questions (FAQs) for recommendations for password selection.

After you have successfully changed your password, you will be able to start the data submission by clicking the Section Names under the “Questionnaire”. You are required to answer all questions under each section.

Welcome to Quality-In-Sights®: Hospital Incentive Program (Q-HIP) Data Collection website!

This website is to support the data submission process from various Q-HIP data sources, including Q-HIP participating hospitals, American College of Cardiology's National Cardiovascular Data Registry (ACC-NCDR®) data to ACC-NCDR; Society of Thoracic Surgeons (STS) Coronary Artery Bypass Graft (CABG) measures and the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS®)

Virginia Health Information (VHI), Anthem-designated Patient Safety Organization, will use this web site to review all data received for each of the Q-HIP hospitals




Procedure for Data Submission
  • You can log into this web site as often as you want. If you have logged into the web site but have no activity for 30 minutes, you will need to re-login to enter and review data. If you have forgotten your password, contact vicki@vhi.org for assistance, if you are a primary Q-Hip user.
  • You can review the scoring criteria under each of the questions. If you have opened a new window for reviewing the scoring criteria OR a link to another web site, please close the new window after your review.
  • In order to save any changes you make to your questionnaire, you must update that information by clicking on the "Continue" button at the bottom of each question.
  • Once you have entered the data, VHI will review and evaluate the information submitted and post their comments for you to review. You can resubmit data in response to deficiencies identified by VHI until any final due dates have passed.
  • If after reviewing the dynamic progress report, you identify errors related to any section of the Q-Hip web tool, please contact VHI directly. Any validated changes will be reflected automatically on the dynamic progress report.
