This website is to support the data submission process from various Q-HIP data sources, including Q-HIP participating hospitals, American College of Cardiology (ACC, the NCDR™ data), and Research Data Inc. (RDI, the survey data). Attention Q-HIP Users: Please note that you will not be able to make any adjustment to the following sections once VHI posts the quarterly reports on this website: ICU Staffing Standards and JCAHO National Hospital Quality Measure Rates. If, after reviewing the quarterly report, you have identified errors related to the above sections, please contact VHI directly, and any new changes will be validated by VHI and be reflected in the following quarterly report. Procedure for Data Submission You can log into this web site as often as you want. If you have logged into the web site but have no activity for 30 minutes, you will need to re-login to enter and review data. If you have forgotten your password, contact vicki@vhi.org for assistance. You can review the scoring criteria under each of the questions. If you have opened a new window for reviewing the scoring criteria OR a link to another web site, please close the new window after your review. In order to save any changes you make to your questionnaire, you must update that information by clicking on the "Continue" button at the bottom of each question. Once you have entered the data, VHI will review and evaluate the information submitted and post their comments for you to review. You can resubmit data in response to deficiencies identified by VHI until any final due dates have passed. You are highly encouraged to submit all hospital related data through this web site. If your facility is unable to submit certain information, the information will be sent to and entered into the web site by VHI. For additional information, view the Frequently Asked Questions (FAQs.) First Time Users You have been assigned a user name and password. Once you enter your user name and password at the top of this screen, click on “Log-in” and the next screen will require you to change the password when first signing on. Please refer to the Frequently Asked Questions (FAQs) for recommendations for password selection. After you have successfully changed your password, you will be able to start the data submission by clicking the Section Names under the “Questionnaire”.
