General Questions

1. What are the purposes of this web site?
There are multiple data sources for Quality-in-Sights®: Hospital Incentive Program (Q-HIP), including data submitted by Q-HIP participating hospitals, cardiac cath and Percutaneous Cardiac Interventions (PCI) data from American College of Cardiology’s (ACC) National Cardiovascular Data Registry (NCDR™), Coronary Artery Bypass Graft (CABG) data from Virginia Health Information (VHI), and patient satisfaction survey data from Research Data Inc. (RDI). This web site will be able to receive all the data related to Q-HIP into one source and presented them to Q-HIP hospitals and other users at various level, so that Anthem-designated Patient Safety Organization (PSO) will be able to review and validate all the data for all Q-HIP hospitals, give timely comments and other communications to each Q-HIP hospital for their performance.

2. Who are the users of this web site?
The users of this web site are as follows:
Q-HIP participating hospitals: each hospital system has established one or more Q-HIP contacts that will function as the web site’s hospital administrator. He or she will have the ability to assign user(s) within his/her hospital to be responsible for data entry, data reviewing, and / or communication for one or more of the Sections assigned. Each Q-HIP contact (the web site’s hospital administrator) will have the responsibility of controlling the users’ access and usage of this web site. The web site’s hospital administrator will be able to review all data related to his/her own hospital(s) only, while the users will be able to review only those sections that have permissions assigned to them by their hospital administrators.

Anthem Health Plans of Virginia, Inc. (Anthem): Anthem will be able to review aggregated hospital level data by section.

Virginia Health Information (VHI): VHI will not only compiles the CABG data for all Q-HIP hospitals that have a open heart program but will also receive hospital level aggregated data for Cardiac cath and PCI from ACC-NCDR™ and patient satisfaction survey data from Research Data Inc. (RDI), on a quarterly basis. VHI will also be responsible for any trouble shootings related to the web site usage. VHI will review and validate all data submitted to this web site from all data sources and for all Q-HIP hospitals. VHI will not be able to enter and / or modify data unless a given hospital is unable to input certain data, then the data will be sent to and entered by VHI.

3. Who designed this web site?
Anthem has contracted with Virginia Health Information (VHI) to design and maintain this web site. VHI is a non-profit organization enacted by Senate Joint Resolution 139 by the Virginia General Assembly. Resolution 139 requested the Secretary of Health and Human Resources to develop a detailed, systematic plan for establishment of a patient level database in Virginia. One of VHI’s missions is to improve the quality of health care for every Virginian. VHI signed a Confidentiality and Code of Conduct Agreement with Anthem to carry out their work and responsibilities for this web site.

4. Will my hospital's information be viewed by other hospitals?
NO.
This web site is set up that only authorized users can enter and / or review data for the hospitals that permission is given (see “Who are the users of this web site” above). Hospital level information will only be available for the given hospital and VHI. However, if your hospital belongs to a hospital system that more than one hospital within that system have participated in the Q-HIP, AND only one Q-HIP contact is established from your hospital system, then the Q-HIP contact person will be able to review all the data for all the hospitals within your hospital system.

5. Will the data be secure and safe on this web site?
YES.
This web site is secured by secure certificate that will be renewed annually. It resides in a dedicated server with fire wall protection for internet connection. VHI also has back up and disaster recovery plan.

Technical Questions


6. What kind of password should I use?
All users should choose passwords that cannot be easily guessed and have a minimum of seven (7) characters.
· Your passwords should not be related to your user name or name in any form.
· Your passwords should contain at least one alphabetic and one non-alphabetic character. Non-alphabetic characters include numbers (0-9) and punctuation.

An initial password will be assigned to a new user name, and will be communicated to the individual through a secure mechanism. Each assigned initial password should be unique.

7. Can two people using the same user name and log in at the same time in two different computers?
NO.
A user will not be allowed to log in if that same user name is already logged in (concurrent sessions are not permitted).

8. I forgot my password and tried to log in several times and when I remembered my password and tried again, I could not log in any more, why?
Only Five failed logon attempts are allowed. Once the limit is reached, the user name will be disabled and the User will be required to get contact with VHI’s Security Officer at Deborah@vhi.org to get his or her account unlocked.
